All children and youth in PLC activities must have a completed Permission/Waiver Form on file at church before they can participate.  Forms may be printed out from our website, or they are available from youth leaders.

In addition, all adult volunteers must fill out a confidential Screening Form and may not serve in ministry to youth until they are approved by the elders.  (Any applicant with a previous history of sexual misconduct or physical assault, or misdemeanor or felony convictions of child abuse (or pleaded no contest) will not be allowed to serve in ministry to youth at PLC.)
